Prior to scheduling an appointment, please contact the office to verify participation in your plan or acceptance of out-of-network benefits.
Upon arrival, please present your insurance card to our reception staff so that we may assess whether or not our providers are in-network with your plan. Please note that acceptance of insurance by a particular company may not include every plan offered by that carrier. You must specify your particular plan with your insurance carrier.
Choosing the best health insurance plan for you and your family is a personal decision, and each patient is ultimately responsible for knowing and understanding one’s own health insurance benefits and restrictions prior to accepting medical services.
We understand that health insurance contractual terms and conditions can be intricate, and we will assist as able with obtaining any necessary pre-authorizations.
If you are of the mindset that the pathway to your personal health and wellness should not be interceded by external third-party restrictions or mandates coming between you and your doctor, and if you wish to “avoid the middle man”, then one of our private practice membership plans may be right for you.

House Calls are available in select geographic areas in the office vicinity for Practice Plus members.
House Calls are offered within a defined radius in reasonable proximity to the office location and generally include but are not limited to the surrounding neighborhoods.

House Calls should be reserved for more urgent needs when you just can’t make it to the office.
House calls are not made in lieu of visits to your local hospital Emergency Department.
If you have a true medical emergency, then you need to call 911 for immediate medical care at the nearest hospital facility.
