These Terms and Conditions govern your participation in the Mountain Medical Practice Notifications messaging program that provides patients with information facilitating their care.
By providing your mobile phone number and joining the Mountain Medical Practice Notifications messaging program, you consent to receiving SMS (Short Message Service) that allows sending short text messages and MMS (Multimedia Messaging Service) that allows sending multimedia content like images, videos, and audio, along with text from Mountain Medical Practice.

Standard message and data rates may apply based on your mobile carrier plan. We are not responsible for charges incurred from your wireless provider.
You can cancel the SMS Service at any time by texting STOP. You will receive a confirmation message and no further texts unless you re-subscribe by texting START or re-opting in through a form.

Carriers are not liable for delayed or undelivered messages. Delivery is dependent on your mobile network operator.

Effective June 1, 2026, Mountain Medical Practice will transition to a fully virtual care model with all appointments conducted via secure telemedicine.
